Job Summary: Under minimal supervision, fill orders for parenteral and enteral medications, monitor patient drug therapies and provide drug information, supervise and direct support personnel.Essential Functions:1. Interprets and processes medication orders (verbal and written) and transcribes to computerized medication profiles accurately. Maintains accurate, complete patient medication profiles.2. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.3. Provides pharmacokinetic consults and drug dosing per organizational protocol.4. Reads, extracts, and interprets information in patient charts accurately.5. Sustains the formulary by minimizing nonformulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection.6. Documents all clinical activities and interventions accurately and completely.7. Converts patients according to protocol from intravenous to oral dosage forms.8. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.9. Accurately dispenses medications.10. Checks pharmacy technicians' work.Minimum Position Qualifications: Education: Graduate of a School of Pharmacy with a BS Pharmacy degree Experience: 2 years of hospital experience or pharmacy residency preferred Training: License / Certification: Currently licensed to practice pharmacy in the state of Texas Preferred Position Qualifications: Pharmacy Residency & Health System Experience
