Position Description Employees in this classification are responsible for providing professional nursing care in an assigned specialty or subspecialty clinic. They perform a variety of general and specialized nursing duties as part of a total health care team to provide the patient with complete evaluations. Employees in this position generally assist the head nurse in the supervision of other clinic personnel and may substitute for this person in the event of his/her absence. May supervise and instruct subordinate nursing personnel. Receives supervision from head nurse or physician, functions independently within guidelines. May supervise ancillary nursing personnel in the performance of certain tasks, and assists in training personnel. Work is performed under usual clinic conditions and requires a high degree of contact with patients, families, and other clinic personnel and outside professionals.

Major/Essential Functions

-Receives symptom based telephone calls directly from the Cardiology Call Center. Appropriately provides guidance to patients regarding urgency of care per clinic protocols.

-Triages walk-in patient who are reporting symptoms and makes quick and accurate assessments determining the urgency of care needed, scheduling appointments with providers or contacting

EMS as needed.

-Reports data, identifies and reports alterations in patient responses to assist in the identification of problems and formulation of goals/ outcomes and patient-centered plans of care in collaboration with patients, their families, and the interdisciplinary health care team.

-For Clinic patients assists in determining the physical and mental health status, needs, and preferences of culturally, ethnically, and socially diverse patients and their families.

-Communicates patient data using EMR and message functionality with decision-making support

to improve patient care.

-Implements teaching plans for patients and their families with health learning needs.

-Participates in the identification of patient needs for referral to resources and facilitates

continuity of care.

-Assists with covering other nurses messages during periods of nurse absences, lunches or meetings. Assist with other nursing duties as needed.

-Assists CHF clinic with obtaining VS, starting IV's or monitoring patients as needed.

-Assist management with bimonthly inventory of medical supplies. Complete bi-monthly

spreadsheet of medical supplies needed.

-Assist CHF nurse and Social Worker with prior authorization forms on CHF patients pre or post

LVAD implantation.

-Inspect, organize and Maintain Sample Medication Room with maintain accuracy of Sample Medication Room. Log in sample medications delivered by mail or drug representative.

-Inspect crash cart for appropriate equipment needed in the event of a patient becoming unstable.

Required Qualifications + Education: + Graduate from an approved accredited nursing program. + License/Certification: + Current RN license in the State of Texas or have the ability to transfer your license to Texas. + Experience: + A minimum of two years of Staff Nurse experience in a specialty area.
